Woodside High School Swim Team

Swimmers of all abilities are welcome. Register by doing the following:

1. Register online for athletic eligibility.
   1. The SUHSD has gone to an online registration and clearance process for athletic eligibility. If you would like a hard copy of the eligibility packet please see Ms. Padilla in the AVP office.
2. Physician signed physicals must be submitted to the athletic office before getting into the pool at the start of our season. Please make sure yours is not about to expire. 
3. All swimmers/athletes must be registered on the school website SportsNet

Physical Form

• This specific form must be completed by a licensed MD to participate in athletics. Chiropractors are NOT accepted for athletic clearance.

You may not get into the pool if:

1. Physical form is expired or not submitted
2. A student is on academic hold
3. A student is on hold for any reason- detentions, fines, etc.
